A direct health care service agreement is not an insurance policy. It covers only the services specified in the signed agreement; goods or services outside the agreement may be billed separately. Apotheosis Health does not bill a third party for services already covered by the Direct Primary Care membership.

Under Maine law, the signed agreement—not this website—must clearly state that direct health care services are not health insurance, do not satisfy a federal requirement to purchase health insurance, and that agreement fees may not be reimbursed by an insurer or applied toward a health insurance policy deductible. Patients may use insurance or other coverage for services outside the membership where applicable.

Apotheosis Health works to keep website pricing and service information accurate and current. Prices, availability, clinical eligibility, and service details may change. Some services are separately priced and are not included in membership.

A listed service or price does not guarantee that a service is appropriate or available for a particular person at a particular time. When provider review is required, scheduling or treatment remains subject to that review.
